A Smile Factory participant is a clinic registered to help complete the treatment of some of the more extensive cases identified at Give Kids a Smile locations. This referral sheet is made available ONLY to clinics hosting Give Kids a Smile events. This is a dentist-to-dentist referral. The Smile Factory provider completes care at a time convenient to the provider and child/parents and the number of appointments is limited. This referral sheet is for clinic use only. Do not give sheet to parents.
